If you are intending to have us bill your health insurance for our services you will need to contact your health insurance company to understand what they will pay and how much you are responsible for. We will need a copy of your insurance card at your initial session. Given higher deductibles that insurance companies are requiring many of our patients have decided to negotiate a self pay rate with us as an alternative.
Please fill out the Intake Form before your appointment and bring it with you. Otherwise plan on coming to your initial appointment a half hour prior to your session to complete this information.
